Slips, Trips & Falls: Environment Audit



Section 1: Flooring & Lighting

Are all floor surfaces (carpets/lino) free from frays, ripples, or loose edges?

Are floor transition strips (door bars) secure and flush to the floor?

Is the lighting in corridors and stairwells bright enough with no flickering or dead bulbs?

Are 'Wet Floor' signs immediately available and used during cleaning or after spills?

Section 2: Obstructions & Exterior

Are corridors and communal walkways completely free from trailing cables and clutter?

Are external pathways and fire exits clear of moss, leaves, or ice (seasonal)?

Are handrails in corridors and bathrooms secure, high-contrast, and clean?

Are residents' footwear choices (e.g., loose slippers) being monitored and flagged to families?

Section 3: Equipment & Furniture

Are all chairs and tables stable (not wobbling) and at appropriate heights?

Are beds kept at the lowest possible height when residents are sleeping?

Are sensor mats/fall alarms positioned correctly and tested for functionality?

Is the nurse call system accessible from a 'fallen' position on the floor?
